Percutaneous Abdominal Solid-Organ Biopsy
Percutaneous abdominal solid-organ biopsy is image-guided needle sampling of a focal lesion or organ parenchyma for histology, cytology, microbiology, immunohistochemistry or molecular testing. Ultrasound is preferred when the target and entire needle path are continuously visible; CT is used for deep, poorly visualized or anatomically complex targets. Technical success is procurement of tissue sufficient to answer the clinical question. Clinically significant hemorrhage is uncommon at approximately 0.5% in a large mixed-biopsy cohort but varies by organ, while reported procedure-related mortality is below 0.05%.
Learning objectives
- Select a biopsy target and route that maximize diagnostic yield while minimizing bleeding and adjacent-organ injury.
- Choose core biopsy, fine-needle aspiration or a combined approach according to the required pathological analysis.
- Apply high-bleeding-risk laboratory and antithrombotic precautions without ignoring thrombosis risk.
- Recognize and manage hemorrhage and organ-specific complications promptly.

Indications
- Determine whether an indeterminate focal solid-organ lesion is benign or malignant when histology will change management.
- Classify or grade a malignancy and obtain tissue for immunohistochemistry, predictive biomarkers or molecular testing.
- Confirm or restage metastatic disease when the result changes treatment intent or regimen.
- Diagnose diffuse parenchymal disease when a percutaneous route is appropriate.
- Obtain tissue for microbiological analysis when infection is suspected and aspiration alone is insufficient.
- Re-biopsy at progression to investigate treatment resistance or obtain trial-mandated tissue.
Absolute contraindications
- No safe percutaneous access route.
- Uncorrectable clinically important coagulopathy.
- Refusal or absence of valid consent.
Relative contraindications
- Haemodynamic or respiratory instability.
- Inability to cooperate or maintain the required position without anaesthetic support.
- Pregnancy when ionizing-radiation guidance would be required.
- Uncontrolled hypertension, severe anaemia or other factors increasing the consequence of bleeding.
- Large-volume ascites or intervening infection that cannot be mitigated.
- Highly vascular lesion or high-risk organ in which an alternative target or route offers lower risk.
Equipment
- Ultrasound unit with colour Doppler and sterile probe cover or multidetector CT with biopsy-planning capability.
- Standard monitoring, oxygen, suction, resuscitation drugs and 18–20G peripheral intravenous cannula.
- Sterile biopsy tray, chlorhexidine-alcohol skin preparation where appropriate, drapes, gauze and dressing.
- 1% lidocaine with 22–25G infiltration needle and longer 20–22G spinal needle for deep local anaesthesia.
- 16–18G automated or semiautomated core-biopsy device with selectable throw appropriate to lesion size.
- Matched coaxial introducer, commonly 17G for an 18G core needle, with depth markings and stylet.
- Optional 18–25G Chiba, spinal or dedicated aspiration needle with 10–30 mL syringes for FNAB.
- 10% neutral-buffered formalin container, glass slides, sterile container or saline for microbiology, and dedicated medium such as RPMI when requested for flow cytometry.
- Optional gelatin-sponge pledgets or slurry prepared before puncture for tract embolization in selected high-risk biopsies.
Procedure steps
- Perform time-out and confirm patient, target, side, indication, antithrombotic plan, coagulation status and required laboratory studies. Confirm how each specimen must be divided and transported before puncture.Pearl: A technically perfect biopsy can still fail if tissue is placed in the wrong medium.
- Position the patient to shorten the safe route, move bowel or lung away and permit comfortable breath-holding. Perform targeted US with colour Doppler or a limited planning CT and mark the skin entry.
- Establish monitoring and intravenous access, prepare a wide sterile field and infiltrate local anaesthetic from skin to the capsule or deep fascia. Make a small skin nick when required for the coaxial needle.Pearl: Capsular anaesthesia is important for liver, kidney and splenic puncture pain.
- Display the complete route immediately before needle insertion. Recheck the planned needle throw so the cutting trough will remain within viable target and will not enter a vessel, duct, collecting system or adjacent organ.
- Advance the coaxial introducer under continuous US visualization or incremental CT guidance with the stylet in place. Use a controlled breath-hold for mobile upper-abdominal organs and stop at the target margin.Pearl: Never advance under US unless the needle tip, not merely the shaft, is confidently identified.
- Confirm that the introducer tip lies at viable tissue and that the intended throw remains entirely safe. Reposition if the target has moved, the tip is in necrosis or vessels have entered the projected path.
- Advance and fire the core device only after checking the throw. Obtain the minimum number of good-quality cores required for diagnosis, redirecting through the coaxial cannula to sample separate viable areas while minimizing dwell time and capsular punctures.Pearl: Coaxial sampling increases tissue yield without repeated organ-capsule puncture.
- Inspect cores for intact tissue and use on-site adequacy assessment when available. Place histology cores in formalin, microbiology material in a sterile non-fixative container and lymphoma material in the pathology-specified medium; label every container at the bedside.
- Remove the core device, then withdraw the coaxial introducer under imaging. Apply external compression when possible and selectively plug the intraparenchymal tract with prepared gelatin sponge when bleeding risk is high and nontarget embolization can be avoided.
- Perform immediate US or CT survey for hematoma, active bleeding, free fluid, pneumothorax or adjacent-organ injury and document the findings. Apply a sterile dressing and transfer the patient for monitored observation with clear escalation criteria.
Complications
- Transient puncture-site or capsular pain
- Hemorrhage, hematoma or hemoperitoneum
- Adjacent-organ injury or bowel perforation
- Infection or abscess
- Acute pancreatitis after pancreatic biopsy
- Catecholamine crisis after adrenal biopsy
- Needle-tract tumour seeding
Exam pearls
- Adequate tissue, not accurate needle placement alone, defines technical success.
- For a subcapsular lesion, a longer oblique tract through normal parenchyma may be safer than a direct capsular puncture.
- Coaxial biopsy means multiple cores through a single organ-capsule puncture and permits tract embolization.
- Never biopsy an adrenal mass until pheochromocytoma has been excluded.
- Target viable enhancing or PET-avid tissue and avoid necrosis.
- Severe pain, tachycardia or hypotension after biopsy is hemorrhage until proved otherwise.
Viva questions
- What defines technical success in percutaneous needle biopsy?
- Technical success is procurement of sufficient, correctly handled tissue to establish the required diagnosis or guide treatment. Needle placement within the lesion alone is not success.
- Why is a coaxial technique preferred for many solid-organ biopsies?
- It permits multiple cores through one capsular puncture, allows redirection within the lesion and provides a route for selective tract plugging. It can therefore improve yield while limiting repeated organ puncture.
- What coagulation values do you target before a solid-organ biopsy?
- Solid-organ biopsy is high bleeding risk. A platelet count of at least 50,000/µL is commonly used; SIR suggests an INR within 1.5–1.8 or lower, while CIRSE-based protocols commonly use 1.5 or lower. Chronic liver disease requires individualized interpretation rather than reflex plasma transfusion.
- How do you select the part of a tumour to biopsy?
- Target viable enhancing or metabolically active solid tissue and avoid central necrosis, hemorrhage and cystic components. Choose the safest lesion that can answer the clinical question, even if it is not the primary tumour.
- What must be done before an adrenal biopsy?
- Confirm that histology will change management, that imaging has not established a benign lesion and that the lesion is hormonally inactive. Pheochromocytoma must be excluded with plasma free or urinary fractionated metanephrines because biopsy can precipitate a life-threatening catecholamine crisis.
- When would you prefer a transjugular rather than percutaneous liver biopsy?
- A transjugular route is considered for nonfocal parenchymal liver biopsy when bleeding risk is unacceptable, particularly with significant coagulopathy or ascites, because bleeding is directed into the venous system. It is not a substitute when a focal peripheral lesion specifically requires targeted sampling.
- How do you manage significant post-biopsy bleeding?
- Recognize pain, tachycardia, hypotension or falling haemoglobin, obtain urgent imaging, resuscitate and reverse antithrombotic effect when appropriate. Ongoing arterial bleeding is treated with selective angiographic embolization; unstable or uncontrolled cases require surgical support.
